Occlusion and the TMJ in Orthodontics

Program Highlights

Correction of a malocclusion to a position in which the condyles are in the correct position can be likened to the proper construction of the foundation of a house. Without it, the house is subject to future instability, as is the malocclusion treated to the incorrect condyle position. This course presents a comprehensive review of the management of orthodontic patients with temporo-mandibular disorders, as well as preventive considerations. A historical review is presented, in which the concept of ideal occlusion is discussed, as well as its relationship to temporo-mandibular disorders. A protocol for diagnosis and treatment planning, as well as splint therapy is discussed. This is followed by a working classification of TMJ disorders, and numerous case examples describing the management of each category within the classification. Additionally, the new Avex appliance systems and information on exciting new adhesives used during both direct and indirect bonding will be discussed in some detail. The arch wire system presently used will also be demonstrated. Selected cases from the other six courses will also be presented intermittently throughout the program.

Program Format

  • Historical Review: Pertinent aspects of the literature will be reviewed concerning occlusion and the temporo-mandibular joint. A detailed discussion of static (centric) occlusion and functional occlusion will be presented. The controversial subject of centric relation will be reviewed. During this lecture a classification concerning the skeletal and dental components of TMJ dysfunction will be presented.
  • Diagnosis and Treatment Planning: The diagnostic process for the TMD patient is reviewed. The benefits and use of the articulator will be reviewed. A three-phase treatment planning process is then presented.
  • Splint Therapy: A discussion of splint therapy will be presented as it relates to management prior to and during orthodontic treatment.
  • Molar Fulcuming Problems: Cases that present with average to high mandibular plane angles frequently show a molar fulcruming problem. Several of these cases will be presented and the mechanics of their treatment will be described.
  • Lack of Vertical Posterior Support: Cases that demonstrate a collapsed vertical dimension will be shown, and methods of treatment of these cases will be described.
  • Management of Anterior Slides and Anterior Posturing: A number of cases in this category will be presented with comments concerning the use and misuse of functional appliances.
  • Correction and Avoidance of Distal Displacement: Distal displacement of the condyle is frequently seen in patients with TMD. This subject is discussed, and a number of cases within this category will also be presented.
  • Management of Lateral Skids and True Skeletal Deficiencies: Discussion and case presentations concerning these two categories will be presented.
  • Selected cases from the other six courses will also be presented intermittently throughout
    the program.
